General Laboratory Requisition - from your Physician

  • This requisition is used to request routine and specialized laboratory testing processed by Alberta Precision Laboratories (APL). Tests are ordered by an authorized healthcare provider and collected at an APL Patient Service Centre, approved clinic, or through an authorized mobile collection service. All specimens are handled in accordance with APL collection requirements, Alberta Health Services standards, and applicable regulatory guidelines to ensure accurate, timely, and confidential results.

    NOTE: These blood tests are performed at IRCC-approved or accredited laboratories and results are submitted through the eMedical system. Laboratory requisition is required from an IRCC-approved Physician Panel.
